Investigators are looking into what went wrong on a skydive jump that left both participants dead, despite their reserve chute deploying. A tear-filled memorial service was held today for murdered Colorado prisons chief Tom Clements, who was killed inside of his home by a former inmate who had ties to a white supremacist gang.
